RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.                     Accept a $300,000 Grant Award from Central California Alliance for Health (CCAH) to support intensive case management funding for medically complex Medi-Cal patients at the Laurel Family Practice clinic; and

b.                     Approve and authorize the Director of Health or Assistant Director of Health to sign a Medi-Cal Capacity Grant Agreement (Grant Agreement), Grant number 0817-MCHD-CS-ICM1,  between Central California Alliance for Health (CCAH) and the County of Monterey, on behalf of the Health Department, Clinic Services Bureau (“County”), in the amount up to $300,000 for intensive case management funding for medically complex Medi-Cal patients at the Laurel Family Practice clinic, effective on December 1, 2017 through January 31, 2020.  

 

SUMMARY/DISCUSSION:

The Health Department Clinic Services Bureau (Clinic Services) operates nine community clinic sites designated as Federally Qualified Health Center Look-Alikes (FQHC-LA), which provide preventative, primary, and specialty medical care services.  Central California Alliance for Health (CCAH) established the Medi-Cal Capacity Grant Program to offer grants to health care providers and community organizations to support efforts that advance the mission of CCAH to provide timely access to quality health care services and to increase Medi-Cal capacity in the service area. 

 

The Intensive Care Management (ICM) Program is a new funding opportunity available under CCAH’s Medi-Cal Capacity Grant Program and is designed to provide an intensive set of services for a specific subset of Medi-Cal patients who require a high level of care coordination.  Many medically complex patients have physical, psychological, and social needs and lack the ability and support to use the health care system effectively.  As a result of their complex needs and a fragmented health care system, these patients often have frequent and avoidable emergency room visits and hospitalizations.   

 

On or around August 8, 2017, the Director of Health notified the Board of Supervisors and the County Administrative Officer of the intent of the Monterey County Health Department to submit a grant application to CCAH’s ICM Grant Program in the amount of $300,000, and on October 27, 2017, Monterey County Health Department was notified by CCAH that it had been awarded the ICM grant for a maximum amount of $300,000.  CCAH’s ICM Grant Award will support funding to add staff and off-set costs to provide intensive case management for medically complex Medi-Cal patients at the Laurel Family Practice clinic.

 

Approval of the recommended action will allow the County to receive the benefit of the CCAH Grant Award and continue to meet the need for quality services and increase healthcare access for our community.

 

This work supports the Monterey County Health Department 2011-2015 Strategic Plan initiative to:  Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.  It also supports one or more of the ten essential public health services, specifically: 7) Link people to needed personal health services and assure the provision of health care when otherwise unavailable, and 8) Assure competent public and personal health care workforce.

 

OTHER AGENCY INVOLVEMENT:

County Counsel, Auditor-Controller, and Contracts/Purchasing have reviewed and approved the Agreement. 

 

FINANCING:

The CCAH Medi-Cal Intensive Case Management Grant is for a maximum of $300,000. While sufficient appropriations are available in FY 2017-18 Adopted Budget, and will be included in FY 2018-19 Proposed Budget for Clinic Services Bureau, Health Department (4000-HEA007); the funds from this grant will help offset a portion of Clinic Services Bureau’s expenditures.
