RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.                     Accept a $1,235,630 Grant Award from the California Department of Public Health (“CDPH”) for the Oral Health Program; and

b.                     Authorize the Director of Health or Assistant Director of Health to execute a Grant Agreement, No. 17-10708, with CDPH in support of the Oral Health Program, in the amount of $1,235,630, for the grant period January 1, 2018 to June 30, 2022.

 

SUMMARY/DISCUSSION:

From 1981 to 2008, Monterey County operated a Happy Teeth children's dental health program which educated school children and increased access to oral healthcare resources (Medi- Cal/Denti-Cal). Happy Teeth provided services to low-income preschoolers and elementary students throughout Monterey County. Services included fluoride education handouts, classroom demonstrations on proper brushing, and educating teachers and involving families in healthy dental hygiene.

 

Despite these successes, no established Local Oral Health Program (LOHP) has been instituted in Monterey County. However, numerous efforts to improve the status of oral health continued. Salud Para la Gente now operates three school-based clinics in Monterey County at Alisal High School, North Monterey County High School, and Pajaro Middle School. In addition, in 2014, Clinica de Salud del Valle de Salinas provided 42,899 dental encounters at five clinic sites and one mobile clinic that serves primarily homeless communities (Office of Statewide Health Planning and Development 2014 Annual Utilization Data).

 

On September 27, 2017, Monterey County Health Department (MCHD) received notification as an awardee through Proposition 56, The California Healthcare, Research and Prevention Tax Act of 2016 (Prop 56). Under the Grant Agreement, Monterey County Health Department Oral Health Program will build capacity and professional expertise, conduct surveillance of social determinants of health, health status, health needs, and healthcare services available among underserved areas and priority populations. Additionally, MCHD will identify assets in the public, private, and non-profit arena to address oral health needs; develop a community health improvement plan and strategies to achieve local oral health objectives; develop an evaluation plan to monitor and assess the progress of the LOHP; and implement evidence based programs to achieve California local oral health plan objectives.

 

The approval of this recommended action will enable collaboration with partners to promote oral health by developing and implementing prevention, policies, and guidelines; integrate oral health in traditional healthcare settings; address chronic disease risk factors such as tobacco use and sugar consumption; and expand existing local health networks to achieve oral health improvements through policy, education, dental care and community engagement strategies.

 

The acceptance of this grant will support the Monterey County Health Department’s 2011-2015 Strategic Plan Initiatives: 1. Empower the community to improve health through programs, policies, and activities; 2. Enhance community health and safety by emphasizing prevention; and 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. Additionally, this work supports three of the ten essential public health services: inform, educate, and empower people about health issues; link people to needed personal health services and assure the provision of health care when otherwise unavailable; and assure competent public and personal health care workforce.

 

OTHER AGENCY INVOLVEMENT:

County Counsel, Risk Management, and the Auditor-Controller have reviewed and approved this Agreement as to legal form, liability and fiscal provisions respectively. The Agreement utilizes standard hold harmless language.

 

FINANCING:

This Agreement is fully funded by the California Department of Public Health.  The Health Department's FY 2017-18 Adopted Budget (001-4000-HEA003-8124) does not include appropriations or estimated revenues for this program.  As necessary, Health will report to the Budget Committee and Board of Supervisors in Spring, 2018, regarding additional appropriations and estimated grant revenues.
