Title: a. Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) or his designee to execute the Fourth Amendment to the Professional Services Agreement (A-11514) with The Regents of the University of California on behalf of the University of California, San Francisco, School of Medicine, Department of Pediatrics (UCSF) to provide neonatology services at NMC, extending the term by twenty-five months (March 1, 2018 to March 31, 2020) for a revised full agreement term of November 1, 2008 to March 31, 2020, with no change to the total not to exceed amount of $2,500,000; and b. Authorize the Deputy Purchasing Agent for NMC or his designee to sign up to three (3) future amendments to this Agreement where the total amendments do not significantly change the scope of work and do not cause an increase of more than ten percent 10% ($105,000) of the original contract amount per each amendment.
