RECOMMENDATION:

It is recommended that the Board of Supervisors:

Approve and authorize the Director of Health or Assistant Director of Health to execute an Amendment No. 2 with extension to Mental Health Services Agreement A-12846 with Telecare Corporation, to add Villa Fairmont Mental Health Rehabilitation Center in San Leandro, CA for the provision of mental health services to Monterey County adults ages 18-65 with a primary diagnosis of severe mental illness; adding a fourth year to the existing term, and increasing the total contract amount by $302,539 for Fiscal Year (FY) 2017-18 and adding $1,363,840 for FY 2018-19, for a new maximum county obligation of $3,182,750 for a new term of FY 2015-16 through FY 2018-19.

 

SUMMARY/DISCUSSION:

This Amendment No. 2 to MHS Agreement A-12846 with Telecare Corporation adds a third program, Villa Fairmont, to the existing FY 2015-16 through FY 2017-18 Agreement, and extends the Agreement one additional FY through FY 2018-19.  A Telecare Mental Health Rehabilitation Center (MHRC), in San Leandro, CA, Villa Fairmont offers two levels of care in a home-like environment to meet the varying needs of clients and payors, Villa Classic and Villa Flex.  Both programs provide services to adults aged 18-65 who live with serious mental illness, and shall make it a priority to serve individuals with psychiatric disabilities that require extensive rehabilitation services beyond those provided in typical sub-acute settings. 

 

Telecare Villa Classic focuses heavily on clients coming out of acute settings, and serves adults and transitional aged youth on a voluntary or involuntary basis.  Villa Classic is staffed at a higher rate than a standard MHRC, which allows for a comprehensive rehabilitation program with a variety of group activities and outings.  Villa Flex focuses heavily on clients with complex needs who require comprehensive services.  Villa Flex typically focuses on clients on Murphy Conservatorship or with other criminal justice involvement, clients stepping down from State Hospitals, or clients at risk for AWOL or self-harming behavior.

 

This Amendment No. 2 to existing Agreement A-12846 with Telecare Corporation provides for two Monterey County placements at each Villa Fairmont program beginning February 1, 2018, and extends the current Agreement an additional year, through FY 2018-19.  In addition, this Amendment sets aside a small portion of funds for the remaining current and last FY of the term for Escort or other 1:1 services as needed.

 

Amendments to existing Agreements are typically retroactive by nature, however, no placements have been made at Telecare Villa Fairmont as of March 1, 2018.  This Amendment is later than anticipated in being presented to your Board as the vendor required 12 weeks to review and sign the draft, and six additional weeks to provide documents as required by County Risk Management and Contracts/Purchasing.

 

This Amendment No. 2 increases the total contract amount by $302,539 for FY 2017-18, and adds $1,363,840 for FY 2018-19, for a revised total contract amount of $3,182,750 for a new term of FY 2015-16 through FY 2018-19.

 

This Agreement contains the County’s standard 30-day “no cause” provision (Section IV, B) and an additional defunding provision (Section IV, D), which provides the County the ability to amend or terminate the Agreement in the event of a reduction and/or termination of funding.

 

This work supports the following Monterey County Health Department 2011-2015 strategic plan initiative: 3) Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.  It also supports one of the ten essential public health services, specifically: 7) Link people to needed personal health services and assure the provisions of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

County Counsel and Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions, respectively.

 

FINANCING:

This Agreement is 100% funded by 1991 Realignment revenues.  The funds for this Agreement ($903,742) are included in the Health Department’s Behavioral Health (HEA012, Unit 8410) Fiscal Year 2017-18 Adopted Budget.  The funds for this Agreement for FY 2018-19 ($1,363,840) will be included in the Health Department’s Behavioral Health (HEA012, Unit 8410) FY 2018-19 Requested Budget.
