RECOMMENDATION:

It is recommended that the Board of Supervisors:

Approve and authorize the Director of Health or Assistant Director of Health to sign Amendment No. 4 to Agreement A-12759 with the California Department of Public Health in the amount of $46,667, for a revised total Agreement of $1,046,667, for the period January 1, 2015 to September 30, 2018, to provide services to local communities with populations at high risk for diabetes and cardiovascular disease under Domains 3 and 4, Health Systems Interventions of the 1305 Supplemental Funds in partnership with area safety net providers.

 

SUMMARY/DISCUSSION:

Monterey County Health Department was approached by the California Department of Public Health (CDPH) to be one of four counties to receive a 1305 Supplemental Funds four-year grant to support cross-cutting approaches to promote health and prevent and control chronic disease and associated risk factors effective January 1, 2015.

 

The grant funds have been used to support work to:

1.                     Implement quality improvement processes in health systems through assessments and planning to increase:

a.                     Electronic health records adoption and the use of health information technology to improve performance,

b.                     The institutionalization and monitoring of aggregated/standardized quality measures at the provider and system level, and

c.                     Engagement of non-physician team members in hypertension and diabetes in health care systems.

2.                     Plan for and conduct training for local health system for expansion of community-clinical linkages (ensuring that communities support and clinics refer patients to programs that improve chronic disease conditions) through assessments, planning, and training to increase

a.                     access, referral, and reimbursement of lifestyle change programs for the prevention of type 2 diabetes;

b.                     Engagement of community health workers in the provision of self-management programs; and

c.                     engagement of community health workers to promote linkages between health systems and community resources for adults with high blood pressure.

 

CDPH approached Monterey County to extend the original Agreement and add additional funding to meet a small number of additional deliverables. The third amendment to the Agreement extended the contract through September 30th, 2018 and, due to CDPH administrative needs, this fourth amendment is adding the additional funding to meet the small number of deliverables, including a Learning Action Network gathering for the healthcare system to discuss using Electronic Health Record systems to support population health management for diabetes and hypertension and two short reports summarizing county efforts in the two goal areas.

 

The acceptance of this agreement supports Monterey County Health Department 2018-2022 Strategic Plan initiative to 1) empower the community to improve health and 3) ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports the following of the ten essential public health services, specifically: 1) Monitor health status to identify and solve community health problems, 4) Mobilize commu8nity partnerships and action to identify and solve health problems, 5) Develop policies and plans that support individual and community health efforts, 7) Link people to needed personal health services and assure the provision of health care when otherwise available, and 8) Assure competent public and personal health care workforce.

 

OTHER AGENCY INVOLVEMENT:

County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to legal form and fiscal provisions. This Agreement is on file with the Clerk of the Board.

 

FINANCING:

All funds for this agreement through FY2019 are covered by the Prevention First Monterey County: 1305 Supplemental Agreement grant from the California Department of Public Health.  There are sufficient appropriations and revenue in the Health Department’s Fiscal Year 2018-19 Adopted Budget (HEA014-8438).
