SUMMARY/DISCUSSION:

The Health Department Clinic Services Bureau (Health Department) operates nine community clinic sites designated as Federally Qualified Health Center Look-Alikes (FQHC-LA), which provide preventative, primary, and specialty medical care services

Central California Alliance for Health (CCAH) is the regional, non-profit Medi-Cal managed care health plan provider for the state Medicaid program, whose mission is to provide timely access to quality health care services to approximately 350,000 Medi-Cal patients across the service area of Monterey, Santa Cruz and Merced counties.

CCAH wishes to provide on-site Healthier Living workshops to eligible patients referred to the program by Clinic Services. The County of Monterey has suitable clinic space and equipment to support these workshops and is willing to allow CCAH to utilize such facilities for the benefit of the patient population.

This work supports the Monterey County Health Department 2018 - 2022 Strategic Plan initiative:  1) Empower the community to improve health.  It also supports one or more of the ten essential public health services, specifically:  3) Inform, educate and empower people about health issues, and 4) Mobilize community partnerships and action to identify and solve health problems.

OTHER AGENCY INVOLVEMENT:

County Counsel has reviewed and approved the Agreement as to form. Auditor-Controller and Contracts/Purchasing have reviewed and approved the Agreement.
