a. Approve and Authorize the Director of Health or Assistant Director of Health to sign a Memorandum of Understanding (MOU) with Oregon Community Health Information Network, Inc. (OCHIN) to participate in the OCHIN BP-MAP (Blood Pressure - Measuring Accurately, Acting Rapidly, and Partnering with Patients) study, a quality improvement initiative focusing on improving blood pressure control, effective upon execution of the MOU through July 31, 2021; and
b. Approve the recommendation of Director of Health or Assistant Director of Health to accept the non-standard indemnification provisions; and
c. Authorize the Director of Health or Assistant Director of Health to sign up to three (3) future amendments to this MOU where the amendments do not significantly change the scope of work; and
d. Accept a clinic impact fee of either $3,000 or $7,000 from OCHIN for participation in the study.

RECOMMENDATION:
It is recommended that the Board of Supervisors:

(OCHIN) to participate in the OCHIN BP-MAP (Blood Pressure - Measuring Accurately, Acting Rapidly, and Partnering with Patients) study, a quality improvement initiative focusing on improving blood pressure control, effective upon execution of the MOU through July 31, 2021; and
b. Approve the recommendation of Director of Health or Assistant Director of Health to accept the non-standard indemnification provisions; and

SUMMARY/DISCUSSION:
The Health Department Clinic Services Bureau (Clinic Services) operates nine community clinic sites designated as Federally Qualified Health Center Look-Alikes (FQHC-LA), which provide preventative, primary, and specialty medical care services. The Health Department has been a member of the OCHIN collaborative since 2007, which allows for access and usage of Epic’s Practice Management (PM) and Electronic Medical Record (EMR) systems.
Through the years, OCHIN has provided valuable health information technology support and services to the Health Department during the course of the Epic PM and EMR System utilization, and continuously offers innovative tools and methods, such as progression towards a paperless system, tracking of quality of care variables, and hosting studies focused to improve health care.
BP-MAP is a quality improvement initiative that supports the implementation of the M.A.P (Measuring Accurately, Acting Rapidly, and Partnering with Patients) program in OCHIN member clinics. The M.A.P. program is an evidence-based protocol developed by the American Medical Association (AMA) to guide the way care teams assess and treat people with high bloodpressure. The study aims to compare the effectiveness of the M.A.P program with Full Support (dedicated practice facilitation) versus a Self-Guided version (online access to M.A.P. materials and orientation webinar) of the program. By participating in this study, clinic care teams will be able to access the latest research, tools, and resources to reach and sustain clinic targets for blood pressure rates.
OCHIN will pay participating clinics either a $3,000 (Self-Guided) or $7,000 (Full Support) clinic impact payment based on which program the clinics are randomly assigned to upon execution of the MOA.
This study is funded by the Patient Centered Outcomes Research Institute (PCORI) that runs from around April 2019 to July 31, 2021. This study is led by the University of San Francisco (UCSF) in partnership with OCHIN, Inc., American Medical Association (AMA) and the American Heart Association (AHA). Study activities have been deemed exempt by the Institutional Review Board (IRB) as there are no research subjects.
Approval of this action allows Clinic Services with the opportunity to improve patient blood pressure rates by gaining access to the latest research, tools and resources developed and piloted by the AMA, which demonstrated significant improvements in blood pressure control in clinics that implemented the M.A.P program.
While this work is not directly in support of a Monterey County Health Department 2018-2022 Strategic Plan, it is in support of one or more of the ten essential public health services, specifically: 1. Monitor health status to identify and solve community health problem; and 3. Inform, educate, and empower people about health issues.
OTHER AGENCY INVOLVEMENT:
County Counsel-Risk Management and Auditor-Controller have reviewed the Memorandum of Understanding and concur with the recommendation.
FINANCING:
OCHIN will be compensating Clinic Services a clinic impact fee of either $3,000 or $7,000 based on the random assignment of a program upon execution of the MOA. While sufficient appropriations are available in FY 2018-19 Adopted Budget for Clinic Services Bureau, Health Department (4000-HEA007); the funds from this payment will help offset a portion of Clinic Services Bureau’s expenditures.
