Approve and authorize the Director of Health or Assistant Director of Health to execute an Amendment No. 2 to Agreement A-13138 with Central Coast Center for Independent Living (CCCIL) to extend the Rapid Rehousing Access provisions of the Whole Person Care Program 2 an additional 18 months, while also extending CCCIL Program 1 counseling and outreach services an additional two fiscal years, increasing the contract amount payable by County to Contractor by a total of $1,003,606 for Fiscal Year (FY) 2018-19 through FY 2020-21, for a new maximum County obligation of $1,612,915, for a revised term of July 1, 2016 through June 30, 2021.

It is recommended that the Board of Supervisors:

Central Coast Center for Independent Living in Salinas, CA is one of a nationwide network of Centers for Independent Living whose philosophy is that people with disabilities should have the same civil rights, options and control over choices in their own lives as do people without disabilities.
