Approve and authorize the Director of Health or the Assistant Director of Health to sign an Agreement with MidPen Resident Services Corporation (MidPen) for case management services for clients in the Whole Person Care program (WPC) qualifying for permanent supportive housing at Moon Gate Plaza, coordinated by the County of Monterey Health Department (Health) for the term retroactive to July 1, 2019 through June 30, 2024 for a total agreement, not to exceed, $929,099.

RECOMMENDATION:

It is recommended that the Board of Supervisors:

Approve and authorize the Director of Health or the Assistant Director of Health to sign an Agreement with MidPen Resident Services Corporation (MidPen) for case management services for clients in the Whole Person Care program (WPC) qualifying for permanent supportive housing at Moon Gate Plaza, coordinated by the County of Monterey Health Department (Health) for the term retroactive to July 1, 2019 through June 30, 2024 for a total agreement, not to exceed, $929,099.

 

SUMMARY/DISCUSSION:

The County of Monterey Health Department (Health) administers the Whole Person Care Pilot Program (WPC), which coordinates physical health, behavioral health and social services for vulnerable Medi-Cal beneficiaries who are high users of multiple systems and continue to have or are at risk of poor health outcomes.  Moon Gate Plaza provides primary case management services and permanent supportive housing to individuals with physical health conditions and psychiatric disabilities who are homeless or at-risk of homelessness.  The program is designed for individuals who need a high level of support to live in permanent housing.

 

Health WPC case management staff will refer very low-income individuals with chronic physical health or serious mental health diagnosis, or both, to occupy up to 20 units in Moon Gate Plaza.  The program will provide intensive case management services in which clients and case managers work together to develop goals to improve client’s functioning in areas of health, education, employment, and daily living skills. 

 

This work supports the Monterey County Health Department 2018-2022 strategic plan initiatives: 1.  Empower the community to improve health through programs, policies, and activities; and 3.  Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.  It also supports one or more of the ten essential public health services, specifically:  3.  Inform, educate, and empower people about health issues.  4. Mobilize community partnerships and action to identify and solve health problems.  7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable. 

 

OTHER AGENCY INVOLVEMENT:

County Counsel-Risk Management and the Auditor-Controller have reviewed and approved this Agreement to legal form and fiscal provisions, respectively.

 

FINANCING:

This Agreement is funded by Whole Person Care and Health Realignment (50% each). There are sufficient appropriations in the Health Department’s FY 2019-20 Adopted Budget for Administration (001-HEA014-8438).  The funds for this Agreement for FY 2020-21 ($180,250), FY 2021-22 ($185,658), FY 2022-23 ($191,227), and FY 2023-24 ($196,964) will be included in the Health Department’s Administration (001-HEA014-8438) Approved Budgets.
