Approve and authorize the Director of Health or Assistant Director of Health to sign State Agreement number 18-10906 with the California Department of Public Health in the amount of $43,000, for the period retroactive to April 1, 2019 through June 29, 2020, to provide services to local communities with populations at high risk for diabetes, cardiovascular disease, and hypertension as part of the 1815 Supplemental Funds; and
b. Approve and authorize the Director of Health or Assistant Director of Health to approve up to three (3) future amendments up to ten percent (10%) ($4,300) of the original contract amount, which does not significantly alter the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of Health or Assistant Director of Health to sign State Agreement number 18-10906 with the California Department of Public Health in the amount of $43,000, for the period retroactive to April 1, 2019 through June 29, 2020, to provide services to local communities with populations at high risk for diabetes, cardiovascular disease, and hypertension as part of the 1815 Supplemental Funds; and
b. Approve and authorize the Director of Health or Assistant Director of Health to approve up to three (3) future amendments up to ten percent (10%) ($4,300) of the original contract amount, which does not significantly alter the scope of services.
SUMMARY/DISCUSSION:
Monterey County Health Department (MCHD) has worked with the California Department of Public Health (CDPH) for five (5) years on the implementation of national cross-cutting approaches on a local level to promote health and prevent and control chronic disease risk factors for diabetes, cardiovascular disease, and hypertension in the areas of electronic health record implementation, team-based care, use of the National Diabetes Prevention Program, and Community Health Workers. The success of the initial five (5) year effort contributed to CDPH choosing Monterey County and MCHD as a partner for a second funding opportunity that CDPH has received from the Centers for Disease Control and Prevention, facilitating community-clinical linkages in these areas of chronic disease prevention as well as creating more local connections with state-wide efforts.
The Agreement will provide funding for MCHD staff to continue through Fiscal 2020, the efforts begun in the first five (5) years of work with CDPH. This will include facilitating local efforts to establish a partnership with one local health center with CDPH to assess organizational capacities to identify people with prediabetes, improving access and participation by the public and providers in CDC-recognized lifestyle change programs and diabetes self-management education and support services, and supporting infrastructure for promoting long-term sustainability of Community Health Workers to expand use of CDC-recognized lifestyle change programs. In addition, staff will work to support connecting CDPH with local implementers of team-based care programs and community health worker programs to improve on chronic disease outcomes in Monterey County.
Health Department staff has been working with CDPH staff for several months to finalize the Agreement. Unfortunately, there were unforeseen delays experienced by CDPH in finalizing the Agreement and MCHD was not in receipt of the final Agreement until July 23, 2019. The Health Department seeks approval to sign Agreement retroactive to April 1, 2019.
This work supports the Monterey County Health Department 2018-2022 Strategic Plan initiative: 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports one or more of the ten essential public health services, specifically:
7. Link people to needed personal health services and assure the provisions of health care when otherwise unavailable, and 8. Assure competent public and personal health care workforce.
OTHER AGENCY INVOLVEMENT:
County Counsel-Risk Management and the Auditor-Controller have reviewed and approved this agreement as to legal form and fiscal provisions, respectively.
FINANCING:
This Agreement is fully funded by the California Department of Public Health. There are sufficient appropriations and revenue in the Departments FY 2019-20 Adopted Budget.
