Title: a. RECOMMENDATION:

It is recommended that the Board of Supervisors:

a. Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) or his designee to execute the Second Amendment to the Professional and Call Coverage Services Agreement with Monterey Anesthesia Professionals (MAP) (A-13387) to provide anesthesia services, extending the term by twelve months (January 1, 2020 to December 31, 2020) for a revised full agreement term of January 1, 2017 to December 31, 2020, and adding $4,340,995, for a revised total not to exceed amount of $15,562,000 in the aggregate; and

b. Authorize the Deputy Purchasing Agent for NMC or his designee to sign up to three (3) future amendments to this Agreement where the total amendments do not exceed 10% ($1,122,100) of the original contract amount and do not significantly change the scope of work.

 

SUMMARY/DISCUSSION:

NMC’s surgical team performs over 3,500 inpatient and outpatient surgeries per year.  It requires a minimum of eight (8) full-time equivalent (FTE) anesthesiologist physicians to provide anesthesia services for NMC’s four operating rooms, including one physician that is immediately available in-house 24 hours a day and one back-up physician that is available on-call 24 hours a day in accordance with American College of Surgeons (ACS) guidelines for a Level II Trauma Center.  Anesthesia services also include airway management (intubations), central line insertion and acute postoperative pain management.

 

Additionally, it requires at least five (5) FTE certified nurse anesthetists (CRNA), including one CRNA that is immediately available 24 hours a day to provide anesthesia services within NMC’s labor and delivery suites. CRNAs may staff one of the four main operating rooms as well.

 

NMC would like to amend the agreement with Monterey Anesthesia Professionals, a California general partnership, to continue to provide exclusive anesthesia services and a physician to serve as medical director, responsible for general administration and oversight of the anesthesia services. NMC has obtained an independent opinion of fair market value supporting the payment terms of this Agreement. 

 

OTHER AGENCY INVOLVEMENT:

County Counsel has reviewed and approved this Amendment as to form.  Auditor-Controller has reviewed and approved this Amendment as to fiscal provisions.  The Amendment has also been reviewed and approved by Natividad Medical Center’s Finance Committee on October 24, 2019 and the Board of Trustees on November 1, 2019.

 

FINANCING:

The cost of this amendment is $4,340,995.  The total not to exceed amount of this Agreement is $15,562,000 for the period January 1, 2017 to December 31, 2020 (48 months).   $4,340,995 is included in the Fiscal Year 2019-20 Adopted Budget.   The remaining amount will be budgeted in subsequent fiscal years.
