RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.  Approve and authorize the Director of Health or Assistant Director of Health to execute an Agreement with the University Corporation at Monterey Bay (Contractor), for the term retroactive to December 01, 2019 through December 31, 2020, for the purpose of establishing and launching the Chinatown Health Engagement Center (CHE) to provide medical, mental health, and ancillary health assessment and treatment, in the amount of $380,000; and

b.  Approve the Director of Health’s recommendation to accept non-standard mutual indemnification provision in the Agreement; and

c.  Approve the Director of Health’s recommendation to accept the change to Royalties and Inventions, providing a method for students at California State University of Monterey Bay (CSUMB) to publish written summaries of their accomplishments and self-evaluations of their work; and

d.  Approve and authorize the Director of Health or Assistant Director of Health to sign up to three (3) future amendments to this Agreement where the amendments do not significantly change the Agreement herein.

 

SUMMARY/DISCUSSION:

Whole Person Care (WPC) pilot is a five-year pilot program authorized under California’s Medi-Cal 2020 waiver to test locally-based initiatives that will coordinate physical health, behavioral health, and social services for vulnerable Medi-Cal beneficiaries who are high users of multiple systems and continue to have or are at risk of poor health outcomes. Through the Department of Health Care Services (DHCS), Health was awarded WPC funds and Health is the fiscal agent and the lead agency charged with implementing the WPC contract agreement, which is in effect through December 31, 2020. One of WPC’s subgrantees is Interim, Inc./CSUMB that has successfully operated the Chinatown Learning Center (CLC), providing life skills, grief and loss counseling, and post-traumatic coping skills to residents in and around Chinatown.

 

Health staff have collaborated with the California State University, Monterey Bay (CSUMB) and the CONTRACTOR to develop the CLC into the Chinatown Health Engagement Center (CHE) to provide WPC enrollees and other homeless individuals with direct services including medical and mental health assessment and treatment, in addition to providing educational assistance. Health regards this exciting new facility as the key structure in its WPC Pilot’s sustainability plan. The new facility will reduce hospital emergency room and inpatient bed days by providing medical and mental health assessment and treatment, and steering unsheltered people into primary care and specialty clinics. This diversion will be strengthened by having a WPC Public Health Nurse Case Managers and Licensed Vocational Nurses, and CSUMB Nursing BSN candidates, Master of Social Work candidates, Physician Assistant students, and Public Health undergraduates all co-located.

 

CHE will be located in 2,000 square feet of lobby, intake area, exam and treatment rooms, sterile room, classroom, and business offices on the ground floor of Moon Gate Plaza, at 22 Soledad Street in Salinas.  Contractor’s letter of intent to sign a lease has been issued with an intended December 2019 move-in. WPC Public Health Nurse Case Managers will be provided with a private office to use when working with Chinatown enrollees and potential enrollees. Contractor has invited physicians, mental health, substance use counselors, housing navigators, and social services providers to use CHE as a location to interact with clients and others seeking services.

 

DHCS approved Health’s proposal to reallocate unused WPC funds to provide one-time funds to develop, manage, operate, and evaluate the CHE. Goals include:

                     Setting up a facility to meet medical, mental health, support services, and educational functions;

                     creating management and operational structures to provide seamless and comprehensive case management services;

                     designing a staffing plan and staff management systems to provide quality services; and

                     designing a financial stability plan to assure reliable continuance of services beyond 2020.

 

Health will require Contractor to submit monthly cost reimbursement invoices for staffing, taxes, benefits, and ongoing operations from the time the Agreement is fully executed through and including the month of December 2020, with a total not-to-exceed County of Monterey obligation not to exceed $380,000.

Contractor exists to enhance the educational program of CSUMB by directly serving students, faculty, and staff and providing services to the public. The Contractor is a 501(c)(3) status nonprofit corporation that is a fully integrated part of the California State University, Monterey Bay campus with grant revenues of approximately $15 million annually. The Contractor complies with federal regulations as stated in 2 CFR 200 and undergoes an annual federal audit as required in subpart F.

This work supports the Monterey County Health Department 2018-2022 strategic plan initiative 3.  Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services.  It also supports five of the ten essential public health services, specifically:  3. Inform, educate, and empower people about health issues, 4. Mobilize community partnerships and action to identify and solve health problems, 5. Develop policies and plans that support individual and community health efforts, 6. Enforce laws and regulations that protect health and ensure safety, and 7.  Link people to needed personal health services and assure the provision of health care when otherwise unavailable.    

 

OTHER AGENCY INVOLVEMENT:

County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to legal form, and fiscal provisions respectively. 

 

FINANCING:

The funds for this Agreement are included in the Health Department Administration Bureau’s FY 2019-20 (HEA014-8438) Adopted Budget.  The funds will be included in the FY 2020-21 Requested Budget.
