RECOMMENDATION:

It is recommended that the Board of Supervisors:

Approve and authorize the Director of Health or the Assistant Director of Health to execute Amendment No. 5 to Agreement No. A-12535, with Seneca Family of Services and the Monterey County Health Department (Health), for the purpose of claiming Federal Financial Participation for Targeted Case Management Services through their Family Ties Program, extending the Agreement an additional three (3) years for a revised full Agreement term of July 1, 2013 through June 30, 2024, and increase the Agreement amount by $200,000 for a total Agreement amount not to exceed $659,000.

 

SUMMARY/DISCUSSION:

The County of Monterey participates in the Medi-Cal TCM Program.  As a county contractor, Seneca provides TCM reimbursable services to their clients.  County oversees the approved TCM Cost Report, and processes claims for payment, for which the County receives a ten percent (10%) administrative fee.

 

The reimbursement or payment rate is determined by the California Department of Health Care Services (DHCS) per an annual cost report.  The County is the TCM administrator and acts as a financial pass through entity for monies from DHCS to Seneca.

 

Health is requesting to amend the current Agreement to extend the term to cover ongoing services for the next three years, for a total of ten years. Approval of this agreement does not inhibit other service providers from also providing TCM case management services in Monterey County using the same model as Seneca.  Health is participating with DHCS and local stakeholders for the new Medi-Cal program CalAIM scheduled to start in January 2022.  Health may request Board of Supervisor approval for other case management contracts as part of TCM or the CalAIM implementation.

 

This work supports the Monterey County Health Department 2018-2022 Strategic Plan Initiatives: 1. Empower the community to improve health; and 2. Enhance community health and safety by emphasizing prevention. It also supports the following of the ten essential public health services, specifically:  4.  Mobilize community partnerships and action to identify and solve health problems; and 7.  Link people to needed personal health services and assure the provision of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

The office of County Counsel and the Auditor-Controller have reviewed and approved this Amendment No. 5 as to legal form and fiscal provisions respectively.

 

FINANCING:

This agreement is 100% funded with Medi-Cal monies through the Targeted Case Management (TCM) program.  There are sufficient appropriations included in the Health Department's (HEA014-8438) Requested Budget for Fiscal Year 2021-22 and will be included in the Health Department’s (HEA014-8438) Requested Budgets for FY 2022-23 and FY 2023-24.
