Report

RECOMMENDATION:

It is recommended that the Board of Supervisors:

a. Approve and authorize the Director of the Department of Social Services to sign Amendment #1 to the agreement with Coalition of Homeless Service Providers for homeless services coordination and Whole Person Care integration to extend the term through December 31, 2021, add $331,145.29 for a contract total of $944,353.29; and

b. Authorize the Director of the Department of Social Services to sign up to three (3) amendments to the agreement that do not exceed 10% ($94,435) and do not significantly change the scope of services.

 

SUMMARY/DISCUSSION:

This amendment allows for the continuation of homeless services coordination in conjunction with the Department of Health Care Services’ Whole Person Care (WPC) program. WPC began as a pilot program through the State Department of Health Care Services with the goal of coordinating health, behavioral health, and social services to improve the health and wellbeing of Medi-Cal recipients.  The State plan is to transition from Whole Person Care to the CalAim Program. The Monterey County Health Department is the lead agency for the County and DSS has partnered with them in the operation of this program. 

 

WPC was originally slated to end December 31, 2020 but has been extended through December 31, 2021 and DSS has been allocated additional funding through this period.  The additional funding allows DSS to extend the agreement with Coalition of Homeless Service Providers and add funding to allow them to continue their work in WPC integration.

 

OTHER AGENCY INVOLVEMENT:

Monterey County Health Department is the lead agency for the County and has partnered with DSS in the operation of this program.  The Auditor Controller and Purchasing have reviewed and approved this agreement. County Counsel has reviewed the agreement as to form.

 

FINANCING:

This entire agreement of $944,353.29 is funded with Whole Person Care grant funds ($442,792.51) and County General Fund Contributions (501,560.78).  There is a 100% required State match which is being met with the County General Fund portion of this agreement. The current amendment for $331,145.29 is funded by $165,572.65 of Whole Person Care and $165,572.65 of County General Fund Contributions and will cover costs approximately from June 15, through December 31, 2021.  There are sufficient appropriations and revenues included in the FY 2020-21 Adopted Budget and 2021-22 Recommended Budget.  There is no change to the Department’s General Fund Contributions.

 

BOARD OF SUPERVISORS STRATEGIC INITIATIVES:

This agreement correlates to the Health & Human Services Strategic Initiative adopted by the Board of Supervisors by providing support services and case management to the County’s most vulnerable population of individuals and families experiencing homelessness.
