RECOMMENDATION:

It is recommended that the Board of Supervisors:

a. SUMMARY/DISCUSSION:

The Health Department seeks authorization to execute the Third-Party Organizer Super User Agreement with the American Heart Association (AHA) to furnish access to the Stroke and Coronary Artery Disease (CAD) modules of the Get With The Guidelines® Program.

Pursuant to the California Health and Safety Code, Section 1797 et seq., the County of Monterey EMS Agency (County) is responsible for emergency medical services (EMS) system coordination, medical oversight, and coordination of the delivery of EMS by provider organizations in Monterey County.

 

The EMS Agency is designated under California law by Monterey County as the local Emergency Medical Services Agency responsible for regulating the Monterey County EMS system. Pursuant to California Code of Regulations, Title 22, Section 100404 (a), the local EMS agency shall develop and implement, in cooperation with other EMS system participants, a system-wide written EMS QI program.  Such programs shall include indicators which address but are not limited to Clinical Care and Patient Outcome. 

 

California Code of Regulations, Title 22, Section 100270.220 authorizes the EMS Agency to develop and implement a stroke critical care system and create a Stroke Critical Care System Plan.  Furthermore, Section 100270.229 (b) states that the EMS Agency shall be responsible for on-going performance evaluation and quality improvement of the stroke critical care system.

California Code of Regulations, Title 22, Section 100270.121 authorizes the EMS Agency to develop and implement a ST-Elevation Myocardial Infarction (STEMI) critical care system and create a STEMI Critical Care System Plan.  Furthermore, Section 100270.127 (b) states that the EMS Agency shall be responsible for on-going performance evaluation and quality improvement of the STEMI critical care system.

 

Designated Stroke and STEMI Centers within Monterey County utilize the Get With The Guidelines (GWTG) Program to facilitate their development of registries of stroke and STEMI patients including data elements pertinent to their care and treatment, provision of performance feedback, and benchmarking of the care of patients against the world’s largest benchmark groups.  This Third Party Organizer Super User Agreement will provide the EMS Agency with access to this data and will also reduce the burden currently placed upon hospitals to provide the data to the EMS Agency via separate processes. This access will allow the EMS Agency to closely monitor care provided in both the pre-hospital and in-hospital setting thus aiding the agency in the Agency’s regulatory and oversight capacities. This agreement is a simple data sharing agreement and as such does not have the standard indemnity or insurance provisions.

 

This work supports the Monterey County Health Departments Strategic Plan Initiatives: 2. Enhance public health & safety through prevention; and 4. Engage MCHD workforce & improve operational functions to meet current & developing population health needs. It also supports two of the ten essential public health services, specifically, 3. Inform, educate, and empower people about health issues; and 9: Evaluate effectiveness, accessibility, and quality of personal and population-based health services.

 

OTHER AGENCY INVOLVEMENT:

The Office of the County Counsel has reviewed this Agreement as to form.

 

FINANCING:

There are no General Fund Impacts resulting from this Board action.  The cost of services to be provided under the proposed Agreement would be covered by CSA-74 special tax (fund 092).  The funds for this Agreement for FY2021-22 were included and for FY2022-23, and FY2023-24 will be included in the Health Department’s Emergency Medical Services Bureau (HEA 006, Unit 8109) respective FY2021-22 Adopted Budget, and FY2022-23, FY2023-24 Requested Budgets.
