RECOMMENDATION:

It is recommended that the Board of Supervisors:

Approve and authorize the Director of Health or Assistant Director of Health to execute Renewal and Amendment No. 2 to Agreement A-15300, with the City of Salinas for housing location and landlord engagement services for individuals experiencing homelessness and for stipends for workers who provide outreach to homeless persons, adding $318,641 to cover additional costs to relocate equipment, purchase additional equipment, and provide additional outreach stipends, for a total Agreement not to exceed, $541,841, for the term January 1, 2021 through December 31, 2021.

 

SUMMARY/DISCUSSION:

The County of Monterey Health Department (“Health”) administers the State of California Whole Person Care (WPC) pilot program that coordinates physical health, behavioral health, and social services for vulnerable Medi-Cal beneficiaries who are high users of multiple systems and continue to have or are at risk of poor health outcomes. WPC subcontracts with community partners to carry out WPC services that additionally ensure that WPC enrollees receive crucial social-emotional support and advocacy while they attempt to reintegrate with health care providers, housing, and employment. The Health Department subcontracts the City of Salinas to provide services for WPC-eligible enrollees.

 

The City of Salinas through the California Emergency Solutions and Housing (CESH) program, provides a variety of services to support unsheltered individuals and those at-risk of homelessness. These services include outreach, landlord engagement, rental and housing relocation assistance, support with lease and tenancy obligations, and linkage to services in the community. In addition, the City of Salinas operates the Navigation and Hygiene Center that provides daily servicing for handwashing and toilet units. The original agreement provided funding for the City of Salinas to and CESH to provide these important and varied services through the end of December 31, 2021. Additional funding became available through budgetary savings at the same time that the City and CESH saw increased need for and provision of services in the Chinatown area. This amendment adds funding to cover all additional services provided and completed through December 31, 2021 and additional funds are covered by state Whole Person Care funds to the County for such services. The Health Department will act as a pass-through at no cost the County. This amendment will increase the original Agreement total from $318,641 to a total not to exceed $541,841, for the term January 1, 2021 through December 31, 2021.

 

This work supports the Monterey County Health Department 2018-2022 Strategic Plan Goal: 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports the following of the ten essential public health services, specifically: 3. Inform, educate, and empower people about health issues, 4. Mobilize community partnerships and action to identify and solve health problems,  5. Develop policies and plans that support individual and community health efforts, 6. Enforce laws and regulations that protect health and ensure safety, and 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

The Office of County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to form and fiscal provisions, respectively.

 

FINANCING:

This Agreement is 50% funded by WPC grant funds and 50% City of Salinas Matching funds.  Appropriations for this Agreement are included in the Health Department's (HEA014-8438) FY 2021-22 Adopted Budget.
