Report

RECOMMENDATION:

It is recommended that the Board of Supervisors:

a.Approve and authorize the Director or Assistant Director of the Department of Social Services to execute Amendment No. 3 to Agreement A-14917 with Central Coast Center for Independent Living to provide long-term services and supports, extending the term to June 30, 2023 and adding $76,051 for a revised contract total of $343,541; and

b. Authorize the Director or Assistant Director of the Department of Social Services to sign up to three (3) amendments to this Agreement where the total amendments do not exceed ten percent 10% ($34,354) of the revised contract amount and do not significantly change the scope of work.

 

SUMMARY/DISCUSSION:

This amendment adds funds, allocated by the California Department of Aging, to support Aging and Disability Resource Center (ADRC) services in Monterey County. Services funded by this agreement include enhanced information & assistance; person centered options counseling; short term service coordination; person centered transition support services; consumer-directed resources for long term support and services (LTSS); and promotion and implementation of a “no wrong door” approach to providing services in a seamless and easy way.

 

OTHER AGENCY INVOLVEMENT:

The Auditor Controller and Purchasing have reviewed and approved this Agreement. County Counsel has approved the Agreement as to form.

 

FINANCING:

This Agreement is funded through the State of California Department of Aging with Aging and Disability Resource Center (ADRC) revenues. There are sufficient appropriations and estimated revenues in the Area Agency on Aging’s 001-5010-SOC10 is included in the FY 2022-23 Recommended Budget.

This Agreement correlates to the Health & Human Services Strategic Initiative adopted by the Board of Supervisors by working toward the goals of improving health and quality of life outcomes through the provision of outreach and supportive services to seniors and adults with disabilities.
