Title: a. Authorize the Chief Executive Officer for Natividad or his designee to execute the Fourth Amendment to the Professional and Call Coverage Services Agreement (A-14-299) with Cardiopulmonary Associates Medical Group (CPA) to provide pulmonary medicine and critical care services, extending the term by twenty-four months (July 1, 2022 to June 30, 2024) for a revised full agreement term of July 1, 2018 to June 30, 2024 and adding $200,000 for a revised not to exceed amount of $600,000 in the aggregate; and b. Authorize the Chief Executive Officer for Natividad or his designee to sign up to three (3) amendments to the agreement where the total amendments do not exceed 10% ($10,000) of the original contract amount and do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Authorize the Chief Executive Officer for Natividad or his designee to execute the Fourth Amendment to the Professional and Call Coverage Services Agreement (A-14-299) with Cardiopulmonary Associates Medical Group (CPA) to provide pulmonary medicine and critical care services, extending the term by twenty-four months (July 1, 2022 to June 30, 2024) for a revised full agreement term of July 1, 2018 to June 30, 2024 and adding $200,000 for a revised not to exceed amount of $600,000 in the aggregate; and

Natividad operates an intensive care unit (ICU) for the care and treatment of hospitalized patients facing life-threatening illness or injury.
