a. Authorize the Chief Executive Officer for Natividad or his designee to execute the Second Amendment to the Professional Services Agreement Alan Smith D.P.M. to provide podiatry services, extending the term by twenty-four months (July 1, 2023 to June 30, 2025) for a revised full agreement term of July 1, 2019 to June 30, 2025, and adding $100,000 for a revised total not to exceed amount of $200,000 in the aggregate; and
b. Authorize the Chief Executive Officer for Natividad or his designee to sign up to three (3) future amendments to this Agreement where the total amendments do not significantly change the scope of work and do not cause an increase of more than ten percent 10% ($10,000) of the original contract amount.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. SUMMARY/DISCUSSION:
Natividad operates the D'Arrigo Family Specialty Services Outpatient Clinic offering over fifteen different specialty care services including podiatry. Natividad has an agreement with Alan Smith DPM to provide podiatry services in the outpatient specialty clinic one day per week.
