| | Claire Hartung, MD Inc. Second Amendment | BoS Agreement | a. Authorize the Interim Chief Executive Officer (CEO) for Natividad Medical Center (NMC) or their designee to execute the Second Amendment to the Professional Services Agreement (A-15977) with Claire Hartung MD, Inc. to provide hospitalist services extending the term of the agreement by twenty-four months (October 1, 2026 to September 30, 2028) for a revised full agreement term August 26, 2022 to September 30, 2028 and adding $100,000 for a revised amount not to exceed $500,000 in the aggregate; and
b. Authorize the Interim Chief Executive Officer for Natividad Medical Center or their designee to sign up to three (3) amendments to these agreements where the total amendments do not significantly change the scope of work, do not exceed 10% ($30,000) of the original contract amount and do not increase the total contract amount above $530,000. | | |

Approve Amendment No. 2 to the Standard Agreement (SA), Multi-Year Agreement #7347, with Terry Oyarzun dba Goats R Us to continue to provide goat grazing services for weed abatement and fire fuel mitigation at various locations within the County, Request for Qualifications (RFQ) #10884, to increase not to exceed maximum by $200,000 to a total of $400,000 and for a revised term from July 31, 2027 to July 31, 2028; and
b. Authorize the County Purchasing Agent or their designee to execute Amendment No. 2 to the SA and future amendments to the SA where the amendments do not significantly alter the scope of work or increase the approved SA amount. | | |
