a. Authorize the Director of Health or Assistant Director of Health to execute Amendment No. 2, effective September 1, 2022, to a Platform License Agreement, No. A-14686, between the County of Monterey and Conduent Health Communities Corporation (CHCC) adding Claritas Consumer Expenditures data to the CHCC Platform and adding $10,000 for a total aggregate amount not to exceed $180,000 for a term of April 15, 2020 through June 23, 2025; and
b. Approve and authorize the Health Director or Assistant Health Director to approve up to three (3) future amendments that do not exceed ten percent (10%) ($17,000) of the original Platform License Agreement amount and do not significantly alter the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Authorize the Director of Health or Assistant Director of Health to execute Amendment No. 2, effective September 1, 2022, to a Platform License Agreement, No. A-14686, between the County of Monterey and Conduent Health Communities Corporation (CHCC) adding Claritas Consumer Expenditures data to the CHCC Platform and adding $10,000 for a total aggregate amount not to exceed $180,000 for a term of April 15, 2020 through June 23, 2025; and

SUMMARY/DISCUSSION:
Conduent Healthy Communities Corporation (CHCC) hosts a Standalone Platform (CHCC Platform) which is a customizable website template system that collects and reports de-identified population health data from multiple sources.
The customization and use of CHCC Platform enables the county and community partners to develop focused strategies, implement programs, and monitor outcomes using the data while maintaining national health accreditation. It will also enable the community to access the information which improves transparency and helps to visually draw associations between social, environmental, and behavioral factors that impact health.
The execution of Amendment No, 2 will enable MCHD to provide hospital utilization, emergency room visits, and mental health data to CHCC who will then de-identify, aggregate, and report the data on the CHCC Platform. This process will involve a secure file transfer protocol, in which only specified users with permissions to access the secured data will receive and utilize the data.
This Amendment No. 2 will also enable MCHD to provide Claritas Consumer Expenditures data on CHCC Platform. The Claritas Consumer Expenditures indicators include percentage estimates of total expenditures for health-related products and services for counties, ZIP codes, census tracts and custom regions by demographic characteristics. This data will be collected directly by CHCC and made available to MCHD on CHCC Platform.
No installation of software locally for MCHD staff is required for data upload, use, or customization of the website. MCHD Staff will utilize the data and website template to provide publicly accessible information on: <https://www.datasharemontereycounty.org/>.
This work supports the Monterey County Health Department 2018-2022 Strategic Plan Goals: 1. Empower the community to improve health; and 2. Enhance community health and safety through prevention. It also supports one of the ten essential public health services, specifically: 9. Evaluate effectiveness, accessibility, and quality of personal and population-based health services.
OTHER AGENCY INVOLVEMENT:
The Office of the County Counsel, Information Technology and Auditor-Controller have reviewed and approved this Service Agreement as to form, technology form and fiscal provisions, respectively.
FINANCING:
Funding for this agreement is included in the Health Department’s (HEA014-8438) FY 2022-23 Adopted Budget and provision for it will be made in the Department’s Requested Budget for the years out.
