a. Authorize the Director of Health or Assistant Director of Health to sign Amendment No. 1 to Agreement No. A-14686, a Platform License Agreement with Conduent Healthy Communities Corporation for the provision of analytics, data visualization tools, customization, maintenance/support, and training for the Healthy Communities Institute Platform System, adding three (3) years and two (2) months for a revised full term of April 15, 2020 through June 23, 2025 and increasing the amount of the agreement by $100,000 for a total aggregate amount not to exceed $170,000; and
b. Approve and authorize the Health Director or Assistant Health Director to approve up to three (3) future amendments that do not exceed ten percent (10%) ($17,000) of the original Platform License Agreement amount and do not significantly alter the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Authorize the Director of Health or Assistant Director of Health to sign Amendment No. 1 to Agreement No. A-14686, a Platform License Agreement with Conduent Healthy Communities Corporation for the provision of analytics, data visualization tools, customization, maintenance/support, and training for the Healthy Communities Institute Platform System, adding three (3) years and two (2) months for a revised full term of April 15, 2020 through June 23, 2025 and increasing the amount of the agreement by $100,000 for a total aggregate amount not to exceed $170,000; and

SUMMARY/DISCUSSION:
Conduent Healthy Communities Corporation (CHCC) hosts a Standalone Platform (CCHC Platform) which is a website template system that collects de-identified population health data from multiple sources. This solution includes a proprietary web based customizable data visualization and mapping tool that enables online analytics and data dashboarding by aggregating information such as health and social factors that impact population health in the community.
CCHC Platform enables the Health Department to facilitate community partnerships and support governing entities by presenting meaningful data through blending clinical with community data. This solution addresses the need for a central system that can collect and organize the data that is publicly available. Data necessary to make informed conclusions relative to social factors that impact health is often in siloed and disparate systems. Siloed data sets across multiple programs/disparate systems pose a challenge in engaging the community stakeholders and aligning resources.
The customization and use of CCHC Platform enables the county and community partners to develop focused strategies, implement programs, and monitor outcomes using the data while maintaining national health accreditation. It will also enable the community to access the information which improves transparency and helps to visually draw associations between social, environmental, and behavioral factors that impact health.
This Amendment No. 1 will enable MCHD to add to the current data set by providing hospital utilization, emergency room, and mental health data to CHCC. All data uploaded to the site for analytics will be de-identified and publicly accessible.
There will be no installation of software locally for MCHD staff to upload and develop the dashboards. Based upon the intended engagement of the vendor and solution by MCHD, it is the recommendation of the Director of Health to request approval of this recommended action.
This work supports the Monterey County Health Department 2018-2022 Strategic Plan initiatives: 1) Empower the community to improve health; and 2) Enhance community health and safety through prevention. It also supports one of the ten essential public health services, specifically: 9) Evaluate effectiveness, accessibility, and quality of personal and population-based health services.
OTHER AGENCY INVOLVEMENT:
County Counsel, Information Technology and Auditor-Controller have reviewed and approved this Service Agreement as to form, technology form and fiscal provisions respectively.
FINANCING:
Funding for this agreement is included in the Health Department’s (HEA014-8438) FY 2020-21 Adopted Budget and will be included in FY 2022-23, FY 2023-24, and FY 2024-25 Requested Budgets, subject to funding availability.
