Title: a. RECOMMENDATION:

It is recommended the Board of Supervisors:

a. Authorize the Purchasing Manager for Natividad Medical Center (NMC) to execute Amendment No. 5 to the Agreement (A-11674) with First Databank for Pharmacy Software Licensing Services at NMC, (no added dollars) for the period January 1, 2005 through December 31, 2014 for a revised total Agreement amount not to exceed $173,717in the aggregate; and
b. Accept non-standard language in the Agreement provided by the vendor and non-standard insurance requirements as recommended by the NMC Chief Executive Officer.

SUMMARY/DISCUSSION:
