RECOMMENDATION:
It is recommended that the Board of Supervisors:
Authorize the Deputy Purchasing Agent for Natividad Medical Center (NMC) or his designee to execute the Fourth Amendment to Professional Services Agreement with Jose Mario Pauda M.D. to provide family medicine services, extending the term of the agreement by twenty-four (24) months (July 30, 2017 to June 30, 2019) for revised full agreement term of July 1, 2011 to June 30, 2019, and adding $40,000 for a revised total not to exceed amount of $140,000 in the aggregate.

SUMMARY/DISCUSSION:
NMC operates the Natividad Immunology Division Outpatient (NIDO) Clinic, a comprehensive primary care clinic serving adults with HIV/AIDS and other infectious diseases. NMC has an agreement with Jose Mario Pauda MD to provide family medicine services in the NIDO clinic one half day per week. NMC wishes to amend the agreement with Dr. Pauda to extend the term an additional twenty-four (24) months in order to continue to provide services without interruption. Dr. Pauda is paid an hourly rate according to the terms of the Agreement, although the amount services provided may increase due to patient volume, the rate of pay remains the same. NMC has obtained an independent opinion of fair market value supporting the payment terms of this Agreement.
