Title: a. RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of Health or Assistant Director of Health to execute a two-year Facility Use Agreement for the term September 1, 2017 to August 31, 2019 with Community Health Innovations, LLC. ("CHI") to use office space on County-leased premises located in Seaside, Marina and Salinas, California to provide Pediatric Wellness Program sessions to eligible Clinic Services' patients at no cost; and
b. Approve and authorize the Director of Health or the Assistant Director of Health to sign up to three (3) future amendments to this Agreement where the amendments do not significantly change the scope of work.

SUMMARY/DISCUSSION:
The Health Department Clinic Services Bureau (Clinic Services) operates nine community clinic sites designated as Federally Qualified Health Center Look-Alikes (FQHC-LA). Under the direction of the County and governance of the Community Health Center Board, the clinics provide preventative, primary, and specialty medical care services.

CHI's Pediatric Wellness Program offers free pediatric wellness coaching services through group education and one-on-one sessions for families referred by their child's pediatrician. The six (6) week curriculum includes group learning on goal-setting, healthy eating and physical activity....
