RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Accept a $21,000 Grant Award from Blue Shield of California Foundation's (BSCF) 2017 Community Health Center Core Support Initiative for the purpose of providing funding in preparation for payment reform programs; and
b. Ratify and approve the acceptance to the Terms and Conditions of Grant No. Core 1708-11647 by the Clinic Services Bureau Chief for the BSCF 2017 Community Health Center Core Support Grant in the amount of $21,000 and effective October 1, 2017 - September 30, 2018.

SUMMARY/DISCUSSION:
BSCF's 2017 Community Health Center Core Support Initiative provides grant funding to help California Federally Qualified Health Centers (FQHCs) prepare for payment reform programs that promote value-based care, including the Payment Reform Pilot Program, for FQHCs to meet the needs of patients by adopting new care models to transform delivery systems through alternative payment methodologies.

BSCF is focused on "improving the lives of all Californians, particularly the underserved, by making health care accessible, effective, and affordable". They work to create greater access to health care and to support system-level improvements in the healthcare safety nets. BSCF is funded by Blue Shield of California, a not-for-profit health plan, and is one of the largest grant providing organizations in California.
