RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Director of the Department of Social Services to sign Amendment #1 to PSA1987 with Unity Care Group, Inc., to provide Short Term Residential Therapeutic Program Individualized Care, adding $352,873, effective April 1, 2018, for a revised contract total of $437,473, and extending the term for one year through June 30, 2019; and
b. Authorize the Director of the Department of Social Services to sign up to three (3) amendments to this agreement, where the total amendments do not exceed 10% ($43,747) of the current amended contract amount, and do not significantly change the scope of work.

SUMMARY/DISCUSSION:
The purpose of this amended agreement is to provide non-Medi-Cal billable, intensive, individualized services for up to two extreme high needs youth identified as having concurrent psychiatric, behavioral, and familial issues. This amendment extends the term of the agreement for one year and will allow the Department of Social Services (DSS) to continue partnering with Unity Care Group, Inc. to address the needs and challenges these youth face related to traumatic stress, attachment, self-efficacy, and hope for the future.
