It is recommended that the Board of Supervisors:

SUMMARY/DISCUSSION:
This Amendment No. 2 to MHS Agreement A-12846 with Telecare Corporation adds a third program, Villa Fairmont, to the existing FY 2015-16 through FY 2017-18 Agreement, and extends the Agreement one additional FY through FY 2018-19. A Telecare Mental Health Rehabilitation Center (MHRC), in San Leandro, CA, Villa Fairmont offers two levels of care in a home-like environment to meet the varying needs of clients and payors, Villa Classic and Villa Flex.
