RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and authorize the Director of Health or Assistant Director of Health to sign Amendment No. 4 to Agreement A-12759 with the California Department of Public Health in the amount of $46,667, for a revised total Agreement of $1,046,667, for the period January 1, 2015 to September 30, 2018, to provide services to local communities with populations at high risk for diabetes and cardiovascular disease under Domains 3 and 4, Health Systems Interventions of the 1305 Supplemental Funds in partnership with area safety net providers.

SUMMARY/DISCUSSION:
Monterey County Health Department was approached by the California Department of Public Health (CDPH) to be one of four counties to receive a 1305 Supplemental Funds four-year grant to support cross-cutting approaches to promote health and prevent and control chronic disease and associated risk factors effective January 1, 2015.

The grant funds have been used to support work to:
1. Implement quality improvement processes in health systems through assessments and planning to increase:
a. Electronic health records adoption and the use of health information technology to improve performance,
b. The institutionalization and monitoring of aggregated/standardized quality measures at the provider and system level, and
c. Engagement of non-physician team members in hypertension and diabetes in health care systems.
