a. Approve and authorize the Contracts/Purchasing Officer or Contracts/Purchasing Supervisor to sign an Agreement with Firato Service Company, Inc., for specialized custodial maintenance services to the Health Department's Clinic Services Bureau for an initial term of January 1, 2021 to December 31, 2023, for an amount not to exceed $1,088,749 and the option to extend the Agreement for two (2) additional one (1) year periods as set forth in RFP#10742; and
b. Authorize the Contracts/Purchasing Officer or Contracts/Purchasing Supervisor to sign up to three (3) future amendments to this Agreement where the amendments do not exceed 10% of the contract amount and do not significantly change the scope of work.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
a. Approve and authorize the Contracts/Purchasing Officer or Contracts/Purchasing Supervisor to sign an Agreement with Firato Service Company, Inc., for specialized custodial maintenance services to the Health Department's Clinic Services Bureau for an initial term of January 1, 2021 to December 31, 2023, for an amount not to exceed $1,088,749 and the option to extend the Agreement for two (2) additional one (1) year periods as set forth in RFP#10742; and

SUMMARY/DISCUSSION:
The Health Department Clinic Services Bureau (Clinic Services) operates ten community clinic sites designated as Federally Qualified Health Centers (FQHCs) which provide preventive, primary, and specialty medical care services.
