RECOMMENDATION:

It is recommended that the Board of Supervisors:

a. Approve and authorize the Director of Health or the Assistant Director of Health to execute Renewal and Amendment No. 1 to the Agreement with the City of Salinas for housing location and landlord engagement services for individuals experiencing homelessness as part of the Whole Person Care Pilot, and stipends for workers who outreach to homeless persons and the management of these services, extending the term of the Agreement seven (7) months to December 31, 2021 and increasing the total obligation by $130,200 for a total Agreement not to exceed, $223,200; and

b. Approve and authorize the Director of Health or Assistant Director of Health to sign up to three (3) future amendments to this Agreement where the amendments do not significantly change the Agreement herein.

 

SUMMARY/DISCUSSION:

The County of Monterey Health Department (“Health”) administers the State of California Whole Person Care (WPC) pilot program that coordinates physical health, behavioral health, and social services for vulnerable Medi-Cal beneficiaries who are high users of multiple systems and continue to have or are at risk of poor health outcomes. WPC services must additionally ensure that WPC enrollees receive crucial social-emotional support and advocacy while they attempt reintegrate with health care providers, housing, and employment.

 

The California Department of Housing and Community Development (HCD), through the California Emergency Solutions and Housing (CESH) Program, has made funding available to the Salinas/Monterey, San Benito Counties Continuum of Care CA-506 (CoC) to provide grants to assist persons who are experiencing homelessness or are at risk of homelessness. The CONTRACTOR’s CESH Program provides funding for a variety of activities to assist persons experiencing or at risk of homelessness as authorized by SB 850 (Chapter 48, Statues of 2018). This agreement allows the CONTRACTOR’s CESH funds to be matched, dollar for dollar, with WPC funding.

 

CONTRACTOR will provide the following CESH activities in support of WPC objectives:

1)                      rental assistance, housing relocation & stabilization services to ensure housing affordability to individuals experiencing homelessness or who are at risk of homelessness;

2)                      flexible housing subsidy funds for local programs that establish or support the provision of rental subsidies in permanent housing to assist homeless individuals & families; and

3)                     operating support for emergency housing interventions including but not limited to navigation centers, street outreach, and shelter diversion.

 

This work supports the Monterey County Health Department 2018-2022 Strategic Plan Initiative: 3. Ensure access to culturally and linguistically appropriate, customer-friendly, quality health services. It also supports the following of the ten essential public health services, specifically: 3. Inform, educate, and empower people about health issues, 4. Mobilize community partnerships and action to identify and solve health problems,  5. Develop policies and plans that support individual and community health efforts, 6. Enforce laws and regulations that protect health and ensure safety, and 7. Link people to needed personal health services and assure the provision of health care when otherwise unavailable.

 

OTHER AGENCY INVOLVEMENT:

The office of County Counsel and the Auditor-Controller have reviewed and approved this Agreement as to form, and fiscal provisions, respectively. 

 

FINANCING:

There is no impact to the General Fund resulting from the approval of this action.  Appropriations are included in the Health Department’s (HEA014 8438) FY 2020-21 Adopted Budget and will be included in the FY 2021-22 Requested Budget.
