Approve and authorize the Director of Health or Assistant Director of Health to execute Renewal and Amendment No. 2 to Agreement A-15300, with the City of Salinas for housing location and landlord engagement services for individuals experiencing homelessness and for stipends for workers who provide outreach to homeless persons, adding $318,641 to cover additional costs to relocate equipment, purchase additional equipment, and provide additional outreach stipends, for a total Agreement not to exceed, $541,841, for the term January 1, 2021 through December 31, 2021.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
Approve and authorize the Director of Health or Assistant Director of Health to execute Renewal and Amendment No. 2 to Agreement A-15300, with the City of Salinas for housing location and landlord engagement services for individuals experiencing homelessness and for stipends for workers who provide outreach to homeless persons, adding $318,641 to cover additional costs to relocate equipment, purchase additional equipment, and provide additional outreach stipends, for a total Agreement not to exceed, $541,841, for the term January 1, 2021 through December 31, 2021.
SUMMARY/DISCUSSION:
The County of Monterey Health Department ("Health") administers the State of California Whole Person Care (WPC) pilot program that coordinates physical health, behavioral health, and social services for vulnerable Medi-Cal beneficiaries who are high users of multiple systems and continue to have or are at risk of poor health outcomes. WPC subcontracts with community partners to carry out WPC services that additionally ensure that WPC enrollees receive crucial social-emotional support and advocacy while they attempt to reintegrate with health care providers, housing, and employment. The Health Department subcontracts the City of Salinas to provide services for WPC-eligible enrollees.
