Title: a. Approve and authorize the Director of Health or Assistant Director of Health to execute Amendment No. 5 to Mental Health Services Agreement A-12775 with Felton Institute for the provision of prevention and recovery in early psychosis services, retroactively to July 1, 2021, to reduce FY 2020-21 amount by $18,132, extend the term for one (1) additional FY, and add $651,740 for FY 2021-22, for a revised total agreement amount of $3,918,608 for the new term of July 1, 2015 through June 30, 2022; and b. Approve and authorize the Director of Health or Assistant Director of Health to approve up to one (1) future amendment that does not exceed ten percent (10%) ($391,861) of the revised Agreement amount and does not significantly alter the scope of services.

RECOMMENDATION:
It is recommended that the Board of Supervisors:
